Care Coordination and Support:
- Tier 1: High-Fidelity Wraparound
- Tier 2: Intensive Care Coordination
Who is eligible?
Children and youth who meet the following criteria:
- Medicaid/Managed Care Organization eligible
- Under the age of 21
- Have a mental health diagnosis/have a Serious Emotional Disturbance (SED) or Serious Mental Illness (SMI)
- Have an Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM+CANS) that demonstrates a need for intensive behavioral health services based on the Illinois Behavioral Health Decision Support Model
Coverage area includes the following counties: Clay, Crawford, Edwards, Effingham, Fayette, Jasper, Lawrence, Marion, Richland, Wabash, and Wayne.
