Conscience Community Network (CCN) Wraparound
Conscience Community Network (CCN) Wraparound is a collaborative initiative in Illinois that connects social service providers to assist families and youth through High-Fidelity and Dually Involved Youth Wraparound programs. By working together, member organizations share innovative ideas and capabilities.
Programs
we offer
High-Fidelity Wraparound: Personalized, team-based support for children, youth, and families facing emotional, behavioral, and mental challenges.
Dually Involved Youth Wraparound: Intensive case coordination and evidence-based treatments for youth in both the child welfare and criminal/legal systems.
Who is eligible?
High-Fidelity Wraparound:
- Determined by the Child and Adolescent Needs and Strengths (CANS) Assessment and is open to individuals with open child welfare cases, ages 0-21.
- Reside in our coverage area.
Coverage area includes the following counties: Coles, Champaign, Douglas, DuPage, Kane, Lake, Moultrie, and Shelby.
Dually Involved Youth Wraparound:
- Youth ages 11-19 involved in both child welfare and the Juvenile Detention Center (JTDC), Department of Corrections (DOC), or have had police involvement and continue to engage in high-risk behaviors.
- Reside in our coverage area.
Coverage area includes the following counties: Cook, DuPage, and Kane.
Cost
There is no cost to youth in this program.
Referrals
One Hope United reviews a list of eligible Illinois DCFS youth on a monthly basis. If cases meet the eligibility criteria, they will be enrolled.
